We craft personalized solutions for each and every situation. Our solution packages and process are adaptable to the level of assistance that you need. We have four different solutions to choose from:
Package One
Health Assessment
Occupational therapist will start with a interview to understand your needs, daily routines, challenges, and goals for independent living. This initial conversation helps tailor the assessment to your unique situation. Throughout the assessment, your therapist might ask you to demonstrate how you perform certain tasks, like getting in and out of the bathtub or reaching for items in high cabinets. It’s simply a way for them to understand your current abilities and identify areas where modifications could help. (This is a Third-Party Service)

Package Two
Home Assesssment
We advise clients with health conditions to undertake both a health and home assessment. If you're preparing for the immediate release from a hospital or rehab center and wish to make your home suitable for release to current home.
Home Assessment: Our assessment thoroughly examines all aspects of safety and accessibility within your home. Upon completing the assessment, we offer recommendations designed to help you create a safe and worry-free living environment for longevity in place.
Package Three
In House Services
Upon receiving your comprehensive home assessment report, we will discuss the findings with you and provide a detailed list of suggested modifications. You will have a variety of options to consider. Additionally, we can provide you with our Nest Wisely trusted home services We can assist with the design and implementation of your modifications. Grab bars installation is the most common service. Not all changes require extensive remodeling; many of our modifications are affordable and can be completed within a single day. We always consider your best interests and budget.
Package Four
Concierge Services
Our concierge services encompass a wide range. We provide recommendations and resouces for caregivers, in-home services, and monitoring devices, relocation services and more. Understanding that this may initially seem daunting, particularly for those not living nearby, we are prepared to intervene and represent your interests. Our expertise in the necessities of independent living ensures that additional support is available when required. With our extensive network of external resources, we can also provide aid income-restricted older adults in locating necessary services. Just for Veteran Services
At Nest Wisely LI, we’re committed to supporting veterans with services tailored to their unique needs. We understand the challenges they face and work to ensure they receive the care, resources, and home modifications they deserve.
One key resource is the HISA grant, which provides lifetime funding for medically necessary home improvements—whether the home is owned or rented—to enhance safety, mobility, and independence.
We assist you every step of the way—from navigating the application process to coordinating and completing the home modifications you need to live safely and comfortably.
